Providers in the fast-paced world of primary care are overwhelmed by the epidemic of preventable chronic disease. Linking community-based resources with primary care clinics is one solution that helps people with or at risk of chronic disease access resources and support to prevent, delay or manage these conditions. 

As part of Minnesota’s Statewide Health Improvement Program (SHIP), a legislative health reform initiative, local public health grantees compiled community resources for local clinics to encourage patient referrals. In addition, local public health conducted key informant interviews about how clinics’ screen, counsel and refer to community resources for increasing access to nutritious foods, prospects for physical activity, and tobacco cessation.
The Minnesota Department of Health’s Community Transformation Grant (CTG) took these findings, aligned them with the National Prevention Strategy, and contracted with Stratis Health. This contract focuses on creating and strengthening community-clinic connections to increase referrals to community-based prevention resources.
