NDEP Webinar Series- Community Health Workers: Their Role in Preventing and Controlling Chronic Conditions

Are you interested in learning about what is going on with community health workers (CHWs) at the national level?
The National Diabetes Education Program and CDC's CHW Workgroup at the National Center for Chronic Disease Prevention and Health Promotion are hosting a webinar that will outline what CHWs are, describe their roles in achieving health equity, and discuss promising practices of the CHW workforce.
The event will showcase the strategies that CDC uses to engage CHWs in chronic condition prevention and control, policy development, and other efforts.   
Speakers will discuss the history and evolution of CHWs at the national level, highlight the experience of organizations implementing the CHW model, and describe lessons learned in working effectively with CHWs.  
A guest community health worker will join this panel of experts, sharing her experiences and perspective of her day-to-day work.

 Presenter Information
J. Nell Brownstein, Ph.D.
Division for Heart Disease and Stroke Prevention, CDC (Retired)
Dr. Brownstein provides scientific research management and technical and evaluation assistance to CHWs. She has also authored CHW-related research papers, literature reviews, and translation briefs.  

Bina Jayapaul-Philip, Ph.D.
Division of Diabetes Translation, CDC
Dr. Jayapaul-Philip provides technical assistance for CHW interventions, performance measurement and evaluation. She co-leads CDC's CHW Workgroup and is a key member of the U.S. Health and Human Services CHW Workgroup.

Alberta Mirambeau, Ph.D., M.P.H., CHES
Division for Heart Disease and Stroke Prevention, CDC/Lieutenant U.S. Public Health Service
Dr. Mirambeau provides evaluation support and technical assistance to state-funded programs, leads evaluation studies at CDC, and co-leads CDC's CHW Workgroup.

Pamela Smart, L.P.N. CHW
Northeastern Vermont Regional Hospital
Ms. Smart is a seasoned community health worker and a supervisor of a team of community health workers that make up the Community Connection Team.  She also serves as the key coordinator of the Community Health Team in Johnsbury, Vermont.
