For assistance with this archived presentation, contact

Meeting Description:
The American Academy of Family Physicians (AAFP) has launched another useful tool to help clinicians address social determinants of health (SDoHs) at the point of care: The Neighborhood Navigator.

Hospitals and health systems can use this free interactive tool during care transitions to connect patients to validated social services in their zip code that can provide help with such needs as food, housing, transportation, employment, and legal or financial issues. The system is equipped to accommodate more than 100 languages and is integrated with Google Maps to offer users step-by-step directions to community resources. Clinicians also can refer patients to a patient-facing version of the tool -- Neighborhood Navigator tool on ( -- to enable them to locate services that meet their social needs on their own. The demonstration of the tool will highlight the key features, basic functionality and explain how it works with the rest of the AAFP’s EveryONE Project’s Social Determinants of Health Toolkit.
