Meeting Description:
Dr. Rash-Foanio will focus on why practices should consider implementing population health management strategies. Population health management strategies have been shown to improve health outcomes, as well as overall efficacy of care; however, making a practice transition to incorporate population health strategies in chronic disease state management can be challenging. This webinar will help participants increase understanding of the tools and strategies on how to implement population health management in practice, and troubleshoot barriers to doing so.

Christine Rash-Foanio, PharmD, BCACP

Dr. Rash-Foanio is a Clinical Pharmacist in Ambulatory Internal Medicine and Managed Care at the UIC Hospital & Health Sciences Center and Clinical Assistant Professor at UIC College of Pharmacy. She provides clinical pharmacy services, including disease state management, within an internal medicine clinic. Her value-based clinical pharmacy model focuses on the diaereses state management of patients with diabetes, hypertension, and cardiovascular disease. Her research areas include medication safety and  population health management.

Dr. Rash is TCPI National Faculty member and serves on the steering committee of the Medication Management and Safety Affinity Group. She graduated magna cum laude from the Ernest Mario School of Pharmacy at Rutgers University. She completed an inpatient pharmacy residency at Hackensack University Medical Center in Hackensack, NJ and a specialty residency in Ambulatory Care at the University of Illinois at Chicago.
