  • Understand keys to success when assisting dialysis patients to kidney transplant
  • Understand the interdisciplinary approach to achieving kidney transplant

Hannah Graves
Hannah Graves is a licensed master social worker in the state of Georgia who has served in the dialysis setting for over two years. Prior to this, Ms. Graves engaged in several different roles in the social work profession experiencing a “Goldilocks phenomenon” where dialysis is “just right” for her. Ms. Graves heralds from Milwaukee, Wisconsin where she received her calling to go in to social work to advocate for social justice in a community considered the number one most segregated city in the United States in 2009. Ms. Graves graduated from the University of Wisconsin Madison with her Bachelor of Arts majoring in Psychology and the University of Wisconsin Milwaukee with her Master of Social Work.
